In the event that the authorized service provider at Malath increases its prices, Malath has the right to replace this service provider with an alternative service provider from within the same medical network, and the company has the right to cancel the service provider according to what it deems in the interest of the company.

To obtain the required compensation, all Malath cash claims must be submitted within a maximum of 30 days from the date of treatment or from the date of arrival in the Kingdom, in addition to duly completing the Malath claim form, and providing a copy of the medical identification card, medical report, doctor’s prescriptions, and medical bill Detailed, diagnostic procedure reports, and detailed receipts (for in-patients).

Persons who are entitled to obtain insurance coverage under the policy:
a) Workers in the sharing economy who are registered as members.
b) Eligible members are given a period of 30 days from the start date of registration for affiliation, and coverage for all individuals begins when the deadline expires.
c) It is not permissible to add any member after the expiry of the period, except for new members.
d) Two additional periods will be granted after the start of the program, provided that the second coverage begins thirty days after the start of the first coverage, and the third is thirty days after the start of the second coverage
e) The maximum number of individuals is 15,000 members, and the affiliation period cannot be extended without obtaining a minimum of 500 members.

All dependents including parents, brothers/sisters, stock holders, service workers, drivers or any member of the employee’s family who are directly sponsored by him are excluded from coverage under this offer.

It is not allowed to change the categories of the insured during the validity period of the policy, and no change is made except in the case of renewing the policy only.

The offer is based on the risk information provided to Malaz, and therefore the offer may be changed if risks are noted to the insured upon receipt of the offer acceptance letter.

If the quotation is accepted, the customer must provide the acceptance letter and a copy, stamped and signed with the following documents:
A) Complete detailed list in excel format for whole numbers
B) A copy of the national identity showing the full information of the members to be added, a copy of the membership and a medical disclosure form signed by the employee concerned.

Where there is any claim paid to a member pre-insured with Malath after the date of deletion of his membership; Compensation will be deposited to the customer to pay that claim.
